My experience with refractive cases right now is somewhat limited; however, I feel very strongly that we should be assessing patients’ preoperative acuity especially with glare at night for documentation purposes and for medical legal purposes. The numbers of high myopes walking in the office who wear contact lenses and say they have no glare - silly. There are very few high myopes wearing contact lenses live without glare and there has been some very large legal verdicts against LASIK surgeons in which patients have described total loss of ability to function at night. I think it is like putting on a seatbelt, safety is first in refractive surgery. Quantitate the patients’ preoperative vision before you do LASIK or an intraocular contact lens of LASIK.
