Though Stereo Optical Co., Inc. offers several packages of vision testing slides, which are well-suited to many common professional, medical, commercial, and government applications, we fully understand that some individuals and organizations do not fit well into any of these profiles. For these Stereo Optical customers we offer the ability to select the appropriate vision testing slides individually from our extensive library, allowing them to take advantage of our industry-standard vision testers and other high-quality products.

If you fall into this category, simply design your own 12-slide package and enter the relevant information on each slide into the Customized Slide Package Form in the order you prefer (see below). Each customized slide package includes a set of personalized record forms and instruction manual.

Our Slide Library

The Stereo Optical slide library is made up of over 150 vision testing slides that can be used to perform almost any contemporary vision test imaginable. With so many slides at your disposal, no vision testing application is out of reach.

Once you have selected the 12 slides you need for your customized package, please enter the relevant information, including slide numbers and descriptions, into the Customized Slide Package Form, available in PDF format. Remember to keep the slides in the order in which you want them arranged. You can submit this form separately or along with your vision tester order.
